At Livingston Place Assisted Living, the Resident Care Manager (RCM) functions as a vital part of the Health Services leadership team to ensure the quality care of all residents. The RCM reports directly to the Director of Nursing and Assistant Director of Nursing; however, the RCM works loosely with all the community management team members to assist with coordinating staff, providing oversight to direct care staff, and coordinating services for the residents. The role requires understanding and commitment to the principles and values of the community and company. This position requires creativity and flexibility with residents and employees who have complex situations and at times this position is stressful. The RCM requires the ability and willingness to train caregivers in tasks required for delivering quality care to the residents of the community. Additionally, this position will be responsible for conducting nursing duties within the scope of nursing practice and assist with the responsibilities of the Director of Nursing in their absence, the ideal candidate will have a minimum of a Licensed Practical Nursing (LPN) license.
Assists with creating and updating Individualized Service Plans as delegated by the Director of Nursing
Assists with Assessments
Assists with Departmental Quality Audits according to the CQI Schedule and as needed
Becomes familiar with the service plan program and the reports it can create
Works with DON to implement individualized service plans as care needs change
Monitors the medication room to ensure organization and cleanliness
Assists Director of Nursing with tracking and recording incident reports. Works with DON on investigation of incidents and accidents as directed
Assists with focus assessments per scope of practice within state licensure
Fills in for Licensed Nurse as needed
Performs evaluations, counsel, discipline, and discharge, if necessary, of all health care staff in conjunction with Director of Nursing and HR Generalist
Follows systems and protocols per Livingston Place at Southern Avenue standards to include but not limited to:
PCC Assessments and Individualized Service Plans
Shift Change and 24-Hour report
Alert charting
Infection control practices
Resident monitoring/documentation of change in condition/Individualized Service Plans
Medication administration and missed medications
Physician Order/Medication Record/Health Record maintenance and reconciliation
Incident and Accident documentation/investigation and notification protocols
Weekly high-risk resident meetings
Other processes are based on state guidelines and regulations
Performs such other tasks as may be required from time-to-time by the management of the property
Other duties as assigned by the Assistant Director of Nursing, Director of Nursing, or Executive Director
EXPERIENCE/EDUCATION
Must be at a mimum a Licensed Practical Nurse (LPN) in Washington DC.
Must possess a current state specific driver's license and appropriate driving record.
Previous experience in senior care and supervising staff is required; minimum 2 years experience in both.
High school diploma or GED preferred.
CPR certification and continued maintenance of certification required .
In-services monthly attendance required.
Flexible availability preferred.

Pay Transparency: Salary to be determined based on factors such as geographic location, skills, education, and/or experience of the applicant, as well as the internal equity and alignment with the team.
For Washington DC this ranges from $66500.00 - $85000.00 plus benefits and retirement program.
